Accident/Injury:
The signatory, being the Mother/Father or lawful guardian of the above-named young, hereby consents to the participation by the young in all age-specific activities and events conducted during TeachME Life Skills Mentor led learning session/s. All young who participate in activities and events within this community will be consistently well supervised with all due diligence and care whilst under the supervision of TeachME Mentors.
Also, as the signatory please advise us of anything you feel that we need to know in reference to each offspring participating to ensure the highest level of care and interaction for your offspring in the space above.
As we are all aware children will be children and in the unlikely event that an incident might occur, every Mentor is required to address the situation bringing a solution as soon as it comes to their attention or as it arises, which is a part of great mentoring at every level. Also, mentors will provide an incident report if necessary. In the changes we are all facing, we endeavour to do all we can within our ability to keep your child safe.
The signatory acknowledges that we are moving into a different mode of life-skill-learnings which their offspring are connecting into and understand that this community is set apart by having smaller group sessions for greater success and connection within the group setting, giving the mentor a great capacity to oversee the group.
